Upvise allows you to configure a global (database-level) PDF header and template-specific PDF headers and footers to suit your form presentation needs. This ensures consistency across exported PDFs while allowing for flexibility when needed per form type.
Setting the Global PDF Header
The global header is applied to all form templates by default unless a specific template override is set.
To configure the global PDF header:
Log in as an Admin User.
Navigate to the Forms module.
Click on Options > Export PDF.
Update the following settings as needed:
Logo
Company Header Text
Font Size
Number of Columns
These settings will apply across all templates unless individually overridden.

Template-Specific Headers and Footers
You can define a unique PDF header and footer for each form template. If not set, the global default will be used.
To set a custom header/footer for a specific form template:
Log in as an Admin User.
Navigate to the Forms module.
Click Templates and open the desired template.
Select Export PDF.
Scroll down to the Custom PDF Header and Custom PDF Footer fields.
Enter the specific content you wish to appear on the exported PDF for this template.
