Upvise Public Forms allow you to make specific form templates accessible to users who do not hold an Upvise license. These forms can be filled out and submitted via a public web URL, making them ideal for use cases such as customer feedback, incident reporting, or external contractor submissions.


Enabling a Form for Public Access

To make a form available to the public:

  1. Navigate to the Form Template in Upvise. (Forms - Templates - Select Template - Sharing)

  2. Go to the "Sharing" section.

  3. Enable the option to make the form public.

  4. Copy the generated URL to your clipboard.

This URL is the public web address where the form can be accessed by anyone with the link.


Creating a QR Code for Your Public Form

A QR code makes it even easier to distribute and access your public form.


Option 1: Using Google Chrome

To generate a QR code directly in Chrome:

  1. Open the copied Public Form URL in a new Chrome browser tab.

  2. Click the browser options (three-dot menu) in the top-right corner.

  3. Navigate to “Cast, Save and Share” (or directly to “Share” depending on your version).

  4. Select “Create QR code.”

  5. Chrome will display a QR code that you can download or copy for use.




Option 2: Using a Third-Party QR Code Generator

If you prefer a more customized QR code, or you’re using a different browser:

  • Use any online QR code generator (e.g., qr-code-generator.com, goqr.me).

  • Paste the Public Form URL into the generator.

  • Customize and download the QR code as needed.

Option 3: Using Sharperlight

If your organisation uses Sharperlight, you can also generate QR codes within that platform by storing and referencing the Public Form URL.

  • Ensure the form remains accessible by not disabling public sharing after generating the QR code.

  • Consider setting up email notifications for submitted forms if you expect external users to submit frequently.

  • Test the public form and QR code on a mobile device before publishing.
